If cancer is in my lymph nodes, does that mean it has spread everywhere?

No. Cancer in the nodes nearby means some cells traveled a short way out of the tumor. It does not mean it has reached the rest of your body.

Plenty of people with cancer in their nodes are still treated with cure as the aim.
