You do not have to give a diagnosis. But telling someone usually opens up rights and flexibility you cannot get without asking. Many people find their workplace kinder than they feared.

Get your treatment schedule in writing first, then have one conversation with the schedule in hand. Ask your team for a letter if that helps, because they write these often.
